Requirement Documents for Business trip/Tourism:

1. Valid/signed passport (valid 6 months with a blank page for visa)
2. Visa application form completed and signed - Print Visa Application Form
3. One passport size photograph
4. For Business trip: Business cover letter from applicant's company (written in letterhead)
5. Green card ( if you are not U.S. citizen and Green card holder, copy is not valid) or Valid re-entry visa to the United States of America   
6. Flight itinerary (For Tourism)
7. For minors under 18 years of age traveling alone, please send a notarized letter   signed  by both parents authorizing this Consulate to issue a visa. If the minor is traveling with only one parent, the other parent must also present (or send) to this Consulate a notarized authorization for the issuance of a visa for the traveling minor
8. A Certificate of vaccination against polio is required for children between ages of three months an six years. If the child cannot be inoculated, a notarized letter from the child's physician will be required

Additional $25.00 communications charge (since transmission of visa application to Brazil is required) applies to nationals of the following countries who are not holders of a green card:
Afghanistan, Algeria, Bhutan, Central African Republic, China, Comoros Island, Croatia, Cuba, Egypt, Eritrea, Ethiopia, Gabon, Ghana, Hong Kong**, India*, Iran, Iraq, Ivory Coast, Jordan, Kampuchea, Laos, Lebanon, Liberia, Libya, Mali, Mauritania, Mozambique, Nigeria, North Korea, Palestinian Auth., Pakistan, Russia*, Sierra Leone, Somalia, Sri Lanka, Sudan, Syria, and Swaziland.

* If living for less than one year in this jurisdiction
** If traveling with a Hong Kong document.

 
Citizens of the following countries do not require a visa to enter Brazil for tourism purposes and a stay of up to 90 days (except Venezuelans for a stay of up to 60 days):
